The form which is provided below can be used to order publications.  We do not yet have an online payment facility, so you will need to send us a cheque or money order to obtain publications directly from us. You have two options: 

1. Print the form [or write us a note with the same information], and mail it to us with the required amount. To print the form, use the <file><print> facility on your browser.

2. Complete the form on-line and email it to us using the "send form" button which is at the bottom of the form. You will still need to send us a cheque before we can dispatch the book.
